How’s Your Health?

One of the most important parts of your health is having a doctor who is on the same page as you.  One tool to help us get on the same page is an internet-based health survey called, “How’s Your Health?”  I request that every patient complete this survey on an annual basis at the time of their annual preventive check-up.  Follow these instructions to complete the survey:

1. Go to website, http://howsyourhealth.org/
2. Clink on the link “Begin Your Health Checkup" and follow the instructions provided. Enter our office code: IP0201
