Our secure online patient portal is now live!  
To sign up for an account, follow these instructions:

  1. Click here for consent form
  2. Read entire document.  Print out last page, sign and bring to the office (or fax to 303-805-5529)
  3. Once we receive your signed consent form, we will send you an email with instructions on how to activate your account
*Access to the patient portal is currently without charge to existing patients.  However, we may be required to charge a small fee for this access in the future.
