How to reach us in case of an after-hours emergency

If you are experiencing a life or limb threatening emergency, please call 911 for immediate medical attention.  For other urgent health related matters, call 303-805-5528 then push 5 to page the on-call doctor.

I need a refill, what do I do?

Ideally, you have remembered to remind us about any needed refills at your most recent appointment.  We try to take care of all of your health-related needs while you are in the office.  This is most efficient for you and us.  If you are in need of refill outside of a regularly scheduled appointment, call your pharmacy and ask them to fax us a refill request.  Even if we have never prescribed the needed medicine for you, the pharmacy can still fax us a refill request.  Having the pharmacy fax us a refill request is the best care we can give you because it confirms the correct name, doseage, and directions of your medicine.

Scheduling Appointments

Please call our office at 303-805-5528 to schedule an appointment. 
If we do not answer the phone right away, please leave a message as we are tending to other patients’ needs in the office.  We will call you back as soon as possible to schedule your appointment.
 

Late for Appointments

If you are more than 15 minutes late for an appointment, we will ask you to reschedule. We will make every effort to schedule you for the next available appointment, but there may be times that this is not possible.  If you are running late, please call the office.  If you miss your scheduled appointment time without calling ahead, you will be asked to pay a cancellation fee.

Cancellation Policy

All appointments require a 24-hour cancellation.  Please provide 24 hours notice for all cancelled appointments, which allows us to see patients that otherwise would not have been able to schedule an appointment.  If a 24-hour notice is not given, you will be charged a cancellation fee of $50.00.  We understand that unforeseen circumstances can arise, so please call us if you won’t make it to your appointment.  Patients who repeatedly miss appointments may be dismissed from the practice.

Payment Policy

You are responsible for any co-payment, coinsurance or deductible at the time services are rendered.  You are also responsible for understanding your insurance plan’s coverage and benefits and for verifying that our office is in-network for your specific plan.  It is also expected that you will pay any remaining balance at the time of service.  There may be an additional service charge if payment is not made at the time of service.  We are glad to work out a payment plan with you if you are having any financial difficulties.

Preventive Visit

An annual preventive exam includes a thorough medical history and physical exam including screening for age-specific health conditions.  Other tests, such as lab work, mammography, colonoscopy, EKG, etc. may be recommended based on your age and family history. 

This visit is generally covered by insurance, but please verify your coverage prior to your appointment.  Because it is considered a preventive visit, insurance will not pay for any other issues to be addressed including chronic medical conditions (diabetes, high blood pressure, asthma, allergies etc.) or acute illnesses (colds, UTIs etc.).  Please plan on addressing these concerns at a separate appointment on a different day if you want your insurance to pay for them.
